PYROTECHNICIAN CREW EMPLOYMENT FORMS

All crew members are required to have the following forms on file file in our office PRIOR to commencing work on a display. If you are uncertain as to which forms you need to complete, please call our office prior to completing new forms.

The following fillable forms can be completed, digitally signed and submitted securely to Western Display Fireworks.

Employee Possessor Questionnaire

Must be completed by ALL crew members once during the period of 2023 through 2026.  Crew members need to complete this form a minimum of 30 days prior to working on a display to allow time for clearance from ATF.

Federal W4

Must be completed by any new crew member who has not previously submitted a W-4 to our office or by any current crew members that would like to update their withholding status.

Please note, Oregon and Idaho residents submitting a Federal W4 are also required to submit an Oregon or Idaho W-4 listing their exemptions.

Oregon W4

Must be completed by any Oregon resident that has submitted a Federal W4.

Idaho W4

Must be completed by any Idaho resident that has submitted a Federal W4.

I-9 Employment Eligibility Verification

Section 1 must be completed by every crew member that has not previously submitted an I-9 to our office. Page 2 provides a list of acceptable documents that MUST be submitted with this form. I-9 forms submitted without copies of acceptable identification documents are considered incomplete.
